In order for Operator personnel to gain access to EIS, they must submit the attached form to studentinformation@tn-asd.org. From there, TDOE will review the form and provide access if it is completed properly.
When completing the form:
1) Please request access as a School User to Data Reports and EIS Production. TDOE rarely fulfills requests at the school-level for other applications.
2) Remember to provide a justification for the request in the Justification section. TDOE returns forms as incomplete when this section remains blank.
